According to ContrastConnect’s guide, the difference between direct and general supervision is determined by the physician’s required availability during medical procedures. Direct supervision requires more stringent oversight: supervisory personnel, which usually comprises radiologists or related subspecialists, must be physically present within the same suite as the procedure taking place and available to intervene immediately if complications arise. As of January 1, 2026, the CMS also now allows direct supervision to be carried out virtually via compliant telecommunications technology; this new rule applies to level 2 diagnostic exams and incident-to services.

Conversely, ContrastConnect’s guide says that general supervision is less restrictive; procedures carried out under this model do not require supervisory personnel to be immediately available, though they must be accessible during the procedure for general guidance. Physicians can satisfy the requirement via phone—an option that is not permitted under the direct supervision model—and other means of remote communication. Additionally, the onus is placed on technologists or nurses to recognize and manage any incidents until physician assistance is available.

ContrastConnect’s guide notes that regulations differ between the models and correspond to the level of supervision required. Per the resource, direct supervision requires constant physician presence, HIPAA- and HITECH-compliant technology, and rigorous documentation. This model is used for all contrast-enhanced exams, which involve iodinated or gadolinium-based contrast agents that pose risks of severe and potentially life-threatening allergic reactions.

Because general supervision requires less oversight, it is often used for lower-risk procedures like plain radiography, bone densitometry, and extremity-only imaging. Nevertheless, the model also requires robust protocols and emergency response systems in place to meet safety standards.

To remain compliant, ContrastConnect says that imaging center staffing plans should include qualified physicians who are available during scheduled contrast procedures. Cross-training for support staff is also vital for emergency situations and telecommunications.

Finally, the guide notes that imaging center layouts should facilitate rapid response to contrast reactions. Emergency protocols should also be documented and updated regularly.
